Recently, Dr. Weidong Wu, chief surgeon of the Department of Gastrointestinal Surgery of General Surgery Center at Shanghai General Hospital, published a comparative study paper about single-incision laparoscopic total extraperitoneal hernia repair (SIL-TES) in the international academic journal Updates in Surgery, “Comparative review of outcomes: singleincision laparoscopic total extraperitoneal sublay (SILTES) mesh repair versus laparoscopic intraperitoneal onlay mesh (IPOM) repair for ventral hernia”. This research was led by Shanghai General Hospital and completed in 2 years by 5 centers in China. It is an original technology in the world.

The ventral hernia occurred on the abdominal wall, especially at the surgical area. It is mainly caused by the patient's weak tissue and poor wound healing and only could be cured by surgery. The ideal surgery for ventral hernia should have the following characteristics: minimal invasive, easy to perform, low-risk, rapid recovery and low cost. Unfortunately, the golden surgery is not existed. In order to find the ideal surgery, Dr. Weidong Wu creatively combined single-incision laparoscopic surgery (SILS) with laparoscopic total extraperitoneal sublay repair (TES), denoted as SIL-TES mesh repair, for ventral hernia therapy and first report this technique on 2020 in China.

This technique eliminated the incidence of visceral trauma caused by trocar settle and adhesiolysis, avoided the occurrence of trocar site hernia, reduced postoperative pain and brought cosmetic effect. Extraperitoneal mesh placement could also avoid complications related with intraperitoneal onlay mesh repair (IPOM). In all, this technique might be according to the requests of ideal ventral hernia surgery.

Thus, collaborated with other hernia centers in China, we together promote this research to provide a faster and cost-effect solution for ventral hernia, a common and frequently-occurring disease in the aging society, and made we Chinese experts voice in the international hernia society.

The General Surgery Center at Shanghai General Hospital is one of the national key clinical specialty, national laparoscopy training base and national general surgeon training base. The Gastrointestinal surgery is one of the pioneers on the field of laparoscopic surgery for gastric cancer and colorectal cancer in China. In recent years, 3D laparoscopy, 4K laparoscopy, Naked eye 3D laparoscopy, Da Vinci robotic surgery was successively carried out by Gastrointestinal surgery and achieved expected clinical effect. As one of these evolution techniques, SILS of Gastrointestinal surgery brought wide influence in Shanghai and even across the country. More than 100 students were received training every year. In addition, Shanghai General Hospital SILS MDT team, including departments of Gastrointestinal surgery, Hepatobiliary surgery, Gynecology, Urology and Thoracic surgery, was founded on 2020 to promote SILS. For the team’s outstanding work, they won the excellent MDT team of SHGH in 2021.

In the future, the General Surgery Center will continue to expand the application of new technology and make great effort to provide high-quality medical services for patients.
