SGH Held the Webinar on Difficult Cases of Spinal Tumors with Rabin Medical Center in Israel

On August 26, Song Dianwen, director of spine surgery of orthopedic clinical medical center of SGH, and Eyal Itshayek, director of spine surgery of Rabin Medical Center in Israel, conducted an academic discussion on the diagnosis and treatment of primary cases of spinal tumors. The two sides shared diagnosis and treatment of difficult cases and carried out in-depth exchanges and discussions. The conference was jointly broadcast online by the media platforms, attracting more than 15,000 audience online.

Vice president Liu Jun attended the meeting and delivered a speech. Song Dianwen, director of spinal surgery, Yin Huabin, deputy chief physician, Dr. Saeed Yassin, spinal surgery of Rabin Medical Center, and Dr. David Samuel Felzensztein reported and discussed surgical treatment cases. Liu Fang, director of the cooperation and exchange department, presided over the meeting.

Liu Jun expressed his appreciation for over ten rounds of online academic exchange jointly held by Rabin Medical Center and Shanghai General Hospital since the outbreak of the pandemic last year. Liu Jun said that the “Cloud” platform allows doctors to share clinical experience and technology, which is particularly important in the context of pandemic. It is hoped that the two sides will explore their common interests, strengthen scientific research and clinical sharing, and carry out continuous and in-depth cooperation and exchanges, so as to make SGH- Rabin International Forum a platform to improve professional ability and international vision. Director Itshayek thanked Liu Jun for his speech and invited colleagues from Shanghai General Hospital to communicate with counterparts of Rabin Medical Center after the pandemic ended.

Song Dianwen shared "Treatment strategies for primary cervical spine tumors". Through vivid diagrams, he explained the difficulty of surgical resection and reconstruction of cervical spine tumors, and proposed the optimal strategy for surgical treatment and the innovation of cervical 1-3 vertebral body titanium mesh reconstruction by domestic scholars in conjunction with the evaluation system of spinal tumors. He then introduced the management strategies and principles of upper cervical spine tumor, lower cervical spine tumor and recurrent cervical spine tumor cases through four cases, Prof. Itshayek was very interested in the case of cervical 2 vertebral body chordoma whole block resection surgery, and he thought that the upper cervical spine tumor surgery was very difficult and risky, and the team of Director Song handled it very well and the prognosis of the patient was excellent.

Saeed Yassin describes the management of a 9-year-old child. The patient had a cervical 2 vertebral body aneurysmal bone cyst that presented with neck pain and limited motion. Yassin described his three-step approach, starting with external support through a cervical brace and arterial embolization in the operative area, followed by posterior resection of the tumor and occipital fusion in the second step, and finally a multidisciplinary team consultation to resect the anterior portion of the tumor from the lateral pharyngeal approach for slope to cervical 4 fusion. The patient was followed up for 5 years and recovered well.

Yin Huabin shared the surgical management of primary giant cell tumor of the spine (GCT). He first introduced the history of primary giant cell tumor of the bone and the evolution of surgical treatment strategies, from curettage in 1938, to complete resection in the 1980s, and then to Professor Boriani's recommendation of whole block resection combined with Enneking staging in the 21st century. The understanding of spinal giant cell tumor of the bone has gradually enhanced and treatment strategies have evolved. At the same time, Denosumab has been approved for the treatment of spinal giant cell tumor and has been shown to have good clinical effect. Yin Huabin emphasized that patients with giant cell tumor of the bone have a long survival period and special attention should be paid to the long-term prognosis of patients with giant cell tumor of the spine.

Dr. David Samuel Felzensztein shares the management of a case of thoracolumbar complex soft tissue sarcoma. Two surgeries were performed to remove the tumor lesion and reconstruct the spinal stability. The postoperative review showed that the spine was stable and no tumor metastasis occurred. Director Song praised the handling of this case as amazing. The exquisite skills, scientific and technological support and the comprehensive strength of multidisciplinary team cooperation embodied by Israeli doctors in the operation are world-class. And Song Dianwen believes that Itshayek team has made the best diagnosis, treatment and treatment for this very difficult case. In the future, it only needs to conduct long-term follow-up for the stability of Endophytes and whether tumors recur, without updating the treatment and repair plan.

Doctors from Rabin Medical Center said that the exchange of difficult cases has "great inspiration for the clinical diagnosis and treatment of spinal tumors". The cases shared by both sides have high academic value. The diagnosis and treatment of difficult cases need to rely on the important role of multidisciplinary cooperation. On the other hand, we should also keep an eye on the patients' postoperative rehabilitation and make choices, which is very important to improve the patients' postoperative quality of life. The spinal surgery team of Shanghai General Hospital has shown strong technical strength and rich clinical experience in the treatment of such difficult cases. Song Dianwen thanked the experts from the spinal surgery team of Rabin Medical Center for participating in the discussion meeting. China has made great progress in the surgical diagnosis and treatment of spinal tumors, and medical technology achievements are emerging. The municipal team has learned innovative technologies for clinical use through this forum, and hopes that the two sides can learn from and cooperate with each other in the future. The two sides said that the next round of exchanges will focus on progress and cooperation in the field of scientific research.
